A five-step self-reflection tool for mapping trauma and stress responses

Most people who experience anxiety, chronic stress, or trauma responses aren't consciously aware of the pattern running underneath — a five-stage cycle that moves from trigger to sensation to urge to action to outcome, often repeating before it's ever named. The Survival Loop is an original clinical framework developed at Clearpath Therapy Group to map that cycle in concrete, somatic terms.

This worksheet walks you through the framework step by step, so you can map a real moment you experienced — what triggered it, where you felt it in your body, what your nervous system wanted to do, and what actually happened. It's designed to be filled out on your own, then brought into a therapy session as a starting point for deeper work.

STEP 01
Trigger / Spark — the specific phrase, event, or cue that started the response.
STEP 02
Sensation & Intensity — where it shows up in your body, and how strong it is.
STEP 03
The Urge — the raw impulse underneath: fight, flight, freeze, or fawn.
STEP 04
The Fork in the Road — what you actually did, and whether it was adaptive or maladaptive.
STEP 05
Outcome & Long-Term Cost — what happened after, and whether the cycle restarted or resolved.
